Advance care planning is becoming more and more a family effort, and that’s great.  Some families even create traditions around holiday discussions about what is important and how one’s wishes can be respected.  The great palliative care physician Ira Byock put it best:  “I have an advance directive, not because I have a serious illness, but because I have a family.”
